Nominate your Vasculitis Patient Hero
Vasculitis is a challenging disease. It’s hard to explain to family and friends what it is like to have the disease. Each patient has their own story to tell. During Vasculitis Awareness Month, the Vasculitis Foundation endeavors to share the patient experience so others may learn from it.
In 2021, we will introduce a series of patient stories told through collectable baseball-type cards. We call them our Patient Heroes. Each card will have a photo of the hero on the front. On the back, the person’s name, type of vasculitis and year he/she was diagnosed, and a quote about the patient’s journey. There will also be a QR Code, when you click on it with your internet-connected device you will see and hear the person telling their story.
This year will be the inaugural year for the collectable cards. Sponsors for Vasculitis Awareness Month will receive packets of cards, the number will vary based on the level of sponsorship. The remainder packet of cards will be used as a premium gift for a minimum $100 donation (until supplies run out).
The campaign is international and is open to patients around the world.
Patients can nominate themselves or be nominated by someone else.
The Nomination Process – We’ve made it easier! Just complete the application. The video and photo are no longer required for the initial nomination.
APPLICATION DEADLINE: Friday, February 19, 2021. Nominations are now closed.
